Abstract

Summery: One of the latest methods for analysis of structural complexity is, Design Structure Matrix which has been used and has been given, lots of attention in academic and scientific centers in recent years. This project was undertaken in order to reveal the structural complexity of Frirooz Abad (Fars Province) Healthcare network. This analytical research has descriptive and realistic dimensions. At first, by studying the functions of present network and organization, the current structure was extracted. Then the direct and indirect dependencies of each sector and units were analyzed. The results of these dependencies in house of quality proof were used in Quality Function Deployment Techniques to generate the current system which is an amalgamation of the two methods mentioned above. Based on our findings, we can give all the responsibilities of the network campaign (Setade Shabake) to the central healthcare system (Markaz Behdasht) and remove it from that system. The mixture of the two mentioned techniques can change the organizational structure of central healthcare system which is dependent on productivity and change it in to a division or a branch. And at the same time, hospital mechanical bureaucracy can change into a professional bureaucracy. However, the organizational structure of emergency centers won’t go through any dramatic changes. Finally, the structure of healthcare network will be of a hybrid one. Combination of the mentioned techniques by modifying the organizational structure without any changes to network functions will lead to a flexible structure that can answer the demands of those who benefit from it.

Abstract

Children with attention deficit hyperactivity disorder have deficiency in many aspects of cognitive functions and quality of life (QOL). The study aimed to examine the relationship between cognitive functions and quality of life in children with Attention Deficit and Hyperactivity Disorder (ADHD). This analytical study is a correlation one. 60 participants were chosen according to statistical formula. Participants were evaluated using the Wechsler Intelligence Scale for Children Revised edition (WISC-R). Their cognitive functions and quality of life (QOL) were assessed by Cognitive Assessment System (CAS), Continuous Performance Test plus Integrated Visual/Auditory (CPT+IVA) and Pediatric Quality of Life inventory (Pads 4.0) Generic Core Scale. After analyzing data a significant correlation was found between attention and school functioning of QOL in all grades, separately. Furthermore, significant correlations were found among full scale attentions of CPT+tIVA test with psychosocial functioning and also total score of QOL, in grade 2 and in grade 5, as well Social Functioning in two grades children. Also significant correlations were found among simultaneous subscale of CAS and emotional, social and psychosocial functioning of QOL, in grade 4. Cognitive functions of children with ADHD have effects on some domains of QOL.

Abstract

Attention Deficit/Hyperactivity Disorder (ADHD) is a neuro-developmental disorder which was diagnosed by three features including: attention deficit, hyperactivity and impulsivity. This study aimed to determine the effect of executive function training based on daily life on reducing symptoms and improving adaptive skills in children with ADHD. 16 children aged 7-10 years old with ADHD were selected by random sampling method from clients of the department of psychiatry in Babol hospital and were divided randomly into experimental and control groups (n=8). Subjects were assessed in two steps at pretest and posttest by SNAP-IV and Barkley Functional Impairment Scale Children and Adolescents (BFIS-CA). The intervention group received 11 training sessions (1 hour in each session) with their mothers. The research findings showed that subjects of the experimental group had significant reduction in posttest scores of SNAP-IV scale than the control group. Significant reduction was observed in the scores of BFIS scale in the experimental group for both Home-school impairment and Community-Leisure impairment subtests. According to findings executive function training based on daily life leads to reduce ADHD symptoms and improve adaptive skills in children with ADHD. It is concluded that executive functions training based on daily life for ease of use and the applicability of its methods can be used in different situations such as home and school for reducing symptoms and improving adaptive skills in children with ADHD.

Background: Multiple Sclerosis (MS) is affected by cognitive and emotional factors. The aim of this study was to investigate the relationship between sensitivity to anxiety and executive functions and the Quality of Life (QoL) of MS patients using path analysis.
Methods: This descriptive correlational research was done on all patients with MS in Gorgan MS Association in 2020. The sample size was considered to be 290 people according to the sample selection formula and based on the values obtained from the previous study selected by simple random sampling. To collect data, the Executive Functions Questionnaire of Nejati (2013), QoL questionnaire of McGuigan & Hutchinson (2004), and Sensitivity to Anxiety Questionnaires of Taylor and Cox (1998) were used. Path analysis with SPSS v. 18 and AMOS v. 23 software was used to analyze the data.
Results: The results showed that the executive functions had a positive relationship with QoL in MS patients and sensitivity to anxiety had a negative relationship with QoL in MS patients. Also, mediating role of the anxiety sensitivity variable in the relationship between executive functions with QoL in MS patients was confirmed (P≤0.01).
Conclusion: The present study indicated the importance of cognitive factors, such as executive functions, and emotional factors, such as sensitivity to anxiety in explaining the QoL in MS patients.
