Methods: A qualitative method with a phenomenological approach was used to explore this study. The samples were diabetes mellitus patients who lived in the Kedungmundu Community Health Center working area and were not experiencing heart or kidney complications about 25 participants. After respondents agreed, in-depth interviews were conducted with respondents using five questions prepared as a semi-structured interview guide. Interview duration about 30-60 minutes.  The data analysis used to analyze the data in this study was seven steps of the Colaizzi phenomenological analytic method.
Results: The research themes in this study consist of diet pattern adjustment, expectations for health information, information on choosing the right medication, need for information on the right type of exercise, biochemical and hemodynamic control, and expectations for health care services.
Conclusion: The themes found are valuable information for health professionals and the government to prepare a national health improvement program to prevent complications from being more optimal and targeted according to the needs of patients in Indonesia and other countries in Asia with the same culture. The development of a personalized care program to reduce the risk of heart and kidney disease complications needs to be done in the future.
