Building stronger primary care matters more than ever as health systems face ageing populations, a surge in noncommunicable diseases, and tighter budgets. Medical schools have a unique chance to shape future doctors for ongoing, community-focused care, but inconsistent and patchy clinical placements keep getting in the way. Iran’s recent move to add a three-month family health internship to the undergraduate curriculum is a step in the right direction. Yet, early data and national surveys show students get wildly different experiences, and medical schools haven’t integrated the internship evenly.
